GLP-1 medicines and your thyroid: the C-cell warning

GLP-1 weight-loss medicines like semaglutide, liraglutide and tirzepatide carry a real FDA boxed warning about thyroid C-cell tumors, but the warning is built on rat and mouse studies, and the labels plainly say it is unknown whether these drugs cause such tumors, including medullary thyroid carcinoma, in humans. The drugs are firmly contraindicated if you have, or have a family history of, medullary thyroid carcinoma or the inherited condition MEN 2. For everyone else, large pooled trial data so far have not shown a clear rise in thyroid cancer, and routine calcitonin or ultrasound monitoring is judged to be of uncertain value. The practical point that often gets missed: GLP-1 drugs slow stomach emptying, which can change how well your body absorbs levothyroxine, so thyroid hormone timing and TSH checks need attention when you start. If you have read the leaflet that comes with a GLP-1 weight-loss medicine such as semaglutide (Wegovy, Ozempic), liraglutide (Saxenda) or tirzepatide (Mounjaro, Zepbound), you may have seen a bold boxed warning about thyroid tumors and felt a chill. That warning is real, it is on every one of these labels, and it deserves to be taken seriously. But it also deserves to be read carefully, because a lot of people stop at the scary headline and miss what the science actually says. The honest picture has four parts. First, the warning is built on rat and mouse studies, where these drugs caused a type of thyroid growth called C-cell tumors. Second, the labels say plainly that it is unknown whether the same thing happens in humans. Third, the drugs are firmly contraindicated if you have, or have a family history of, medullary thyroid carcinoma (MTC) or a rare inherited condition called Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Fourth, there is a separate, very practical issue that often gets missed: GLP-1 medicines slow down your stomach emptying, and that can change how well your body absorbs levothyroxine, the thyroid hormone pill.
